• Reducing Total Cholesterol: In addition to lowering LDL cholesterol, Simvastatin and Ezetimibe can also reduce overall levels of total cholesterol in the blood.
• Increasing HDL Cholesterol: While the primary focus is on reducing LDL cholesterol, this medication combination can also have a modest effect on increasing levels of HDL cholesterol, which is considered “good” cholesterol because it helps remove LDL from the bloodstream.

Simvastatin:

• Mechanism of Action: Simvastatin is a type of medication known as a “statin” or HMG-CoA reductase inhibitor. It works by inhibiting an enzyme called HMG-CoA reductase in the liver. This enzyme is crucial in the production of cholesterol in the body. By inhibiting this enzyme, simvastatin reduces the production of cholesterol, particularly low-density lipoprotein (LDL) cholesterol, which is often referred to as “bad” cholesterol.
• Pharmacology:
  • Absorption and Distribution: After oral administration, simvastatin is absorbed from the digestive tract and undergoes a significant first-pass metabolism in the liver, where it is converted to its active form, simvastatin acid.

Ezetimibe:

• Mechanism of Action: Ezetimibe is a cholesterol absorption inhibitor. It works in the small intestine, specifically at the brush border of the enterocytes. Ezetimibe inhibits the absorption of dietary cholesterol, as well as cholesterol that is reabsorbed from bile, by blocking a protein called the Niemann-Pick C1-like 1 (NPC1L1) transporter. By doing so, ezetimibe reduces the amount of cholesterol that enters the bloodstream.

  • Absorption and Distribution: Ezetimibe is well-absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 1-2 hours after oral administration.
  • Metabolism: It undergoes glucuronidation in the liver to form the active metabolite ezetimibe glucuronide, which is responsible for its cholesterol-lowering effects.

Simvastatin & Ezetimibe Tablets – Side-effects

Common Side Effects:

1. Headache: Some individuals may experience mild to moderate headaches.
2. Digestive Issues: This can include constipation, diarrhea, stomach pain, nausea, or flatulence.
3. Muscle Pain or Weakness: This is more common with statin medications like simvastatin. In rare cases, it can lead to a condition called rhabdomyolysis, which is a severe breakdown of muscle tissue.
4. Fatigue or Weakness: Some people may feel tired or weak, particularly when starting the medication.

Less Common but Potentially Serious Side Effects:

• Liver Problems: In rare cases, statins like simvastatin can lead to liver enzyme abnormalities. These are typically monitored with blood tests.
